Dear occupational therapist or certified occupational therapy assistant,
We are a team composed of University of Michigan occupational therapists, researchers, and stakeholders, as well as occupational therapy student researchers from Eastern Michigan University in the United States. We are developing a gaming device to play adapted video games for upper extremity rehabilitation. As an occupational therapy provider, we are highly interested in gathering licensed occupational therapist and occupational therapy assistant input from across the world.

The survey is anonymous and uses the REDCap system for gathering data for clinical research. It is HIPAA compliant and highly secure. We are going to have a raffle for a USD$50 visa gift card. A name and email is only required at the end of the survey, should a respondent want to be eligible for the gift card drawing. We will discard your name and email after we select a winner.
Finally, a respondent is free to share this survey link with other occupational therapists and occupational therapy assistants.
